We are currently unable to book appointments via the internet and e-mail. We apologize for any inconvenience this may cause you.

If you are a new patient to our practice, please have your insurance information available. If you have any questions regarding your coverage or co-payment(s), please ask our receptionist when you make your appointment.

We kindly request you to arrive on time or early for your appointment to find parking and to fill out necessary paperwork.

For our new patients: Please remember to bring in your glasses and contact lens information if you have them available. If you would like to us to request this information on your behalf from your previous optometrist, please let us know when you book your appointment.
